VAWA Forms Updated and Available for MORs

HUD has published updated Violence Against Women Act (VAWA) forms for use by Contract Administrators and HUD’s Office of Multifamily Housing staff in conducting Management and Occupancy Reviews (MORs). The new forms replace previously expired versions and are now available on HUD Forms webpage.

For multifamily programs, the U.S. Department of Housing and Urban Development requires owners and agents to provide tenants with HUD-5380 (Notice of Occupancy Rights) and HUD-5382 (Certification Form), which are the primary documents reviewed during MORs. Owners must also maintain an emergency transfer plan consistent with VAWA requirements (HUD-5381 provides a model). Additional forms—HUD-5383 (Emergency Transfer Request) and HUD-5384 (Lease Bifurcation Certification)—are used only in specific circumstances.
